Is this how the effect is done naturally in lighting with minimal post processing?  Obviously there is some D&B on images but baby oil is common I assume?

but in any case I think most of the effect is done in post anyway... you just need a good retoucher wink

I see pictures like this a lot, and I always wonder why the digital artist didn't treat the neck and shoulders in a manner similar to the face.

The whole image has strong shadows so i would remove that a little bit everywhere.
If you remove the shadows in shoulders and neck will not look good cause it has shadows in the head as well.
