hello all,

I am a makeup artist in canada. I would like to know how to make a contract/consent form for models or brides. I am currently working on my portfolio so I am basically working with photographers and models for Trade for shoots. I also have some models who are under 18. Can you please tell me how to make a form. I dont want to end up with any trouble.

The first thing to think about is what type of things you'd like to protect yourself from. This will make a good basis for your contract.
Have you any thoughts on that note?
