If I had to guess I would say it is a beauty dish located dead centre in front of the model and angled down and  a shovel modifier on the light pointed at the background at least 2/3 stop brighter than the main beauty dish.

The shadow transition under the lip looks too gradual to be a silver umbrella and the specularity on the rest of the face would suggest it is not soft enough to be a softbox.

I am totally guessing though so hopeful other people with comment. De-constructing image can be fun and show you different ways to create images.

Short lighting him with the beauty dish to imitate the sun, and using the two head for clothing texture and fill on his face without hitting the vehicle which is being nicely lit by the setting sun

Three lights:  All Elinchrom Ranger Quadra A-Heads.  The overhead is a 24x24 Lastolite collapsible softbox with an 8" mask on the front, oriented vertical to the model.  The other two lights are reflectors with 30 degree grids behind and to the sides of the model (if the model was at the center of a clock facing 6 o'clock, the two lights are at 10 and 2 o'clock). 

Overhead light metered to f/9, side lights metered on stop below.  The reflector is a Lastolite Tri-reflector adding fill below the model and to fill the front sides.

Canon 7D, 70-200mm f/2.8 IS L II, @ 160/sec, f/9, ISO 200.

Finished with Nik Silver Efx Pro.

bts: Decided to feather Mola bd to better blend with the background strobe's sun effect. Looks more like natural light. Did recent shoot with Mola pointed directly at model and didn't blend as well

MOLA is a beauty dish,not sure of answer to second question,maybe just light from gelled backlight wrapping round face?? Message the photog and ask.

Came up with an idea for a shoot to do for when I re-do my website. Curious on how I would get similar lighting to the photos below.

I know the second image is a white backdrop which I have, but not sure about the others.
