The lighting, and the emotion on his face and his general body language conveys some type of hardship/strife/regret he is dealing with, but at the same time there is that hand... at the bottom... placed in that place. Sexual frustration? Past notions about a lover? Stalker fever, "I can't have the girl but I'm going to touch myself and think about her."
