Lighting:
Model recently bleached her hair blonde for a movie role
2 small gridded softboxes from the rear, each off by about 45deg to create rim-light
1 large octobox from the front, high and to the right as key light
A white reflector low and left to fill in shadows
Post:
2 B&W conversion layers (masking eyes & lips on one to enhance contrast)
D&B layer
Separate High- and Low-pass layers for skin retouching
