It was done with layer masking/blending in CS4 from multiple developed version of the same photo. 3 Light setup, with the bare key behind the model camera right. Front two lights through umbrellas to balance. I think I was at f/16 and 1/15 to get some background ambiance. I also had to color blend another developed version with florescent white balance cus I was too lazy to put filters on my lights.
