Mark Wangerin! Damn good photographer! I paid him to teach me for a day so I'll offer what little I can remember. Use the sun as a light source behind and to an angle to the model. Match the sun with your key on the other side of the model (relative to the sun). Use a bare bulb flash about three to four stops under the key to add "a subtle but soft glow." Kickers and/or accent lights for the background should be about match the key or be no more than one stop under the key. 

I don't remember if he uses Nik Color Effects or Nik Silver Effects. It's one of the two I think. Plus judacious use of the dodge and burn tools in Photoshop to shape and draw the eye.

He does offer lighting workshop, which having taken one myself, I can personally recommend. He's also coming out with a book and a DVD some time in the next few months or so.
