If you have a Canon, it's on the disk that comes with the camera.

If you have PS, there are many ways you can do it, search Youtube.

Normally I would create a blank canvas in PS and drag and drop images onto it. That's the quickest way I can think of.

I use PTGui.  I like it well enough, and would recommend it.  I also have a gigapan and their software.

I would say PTGui Pro ($) is my favorite, although Hugin (free) isn't bad and will do in a pinch. Both are based on the program Panorama Tools.

Used Autopano Pro and love the results. It's capable to make panorama's beyond the JPEG size (30.000 pixels).
