8) Pay (please just don't ask me what my rates are...)
9) Travel Compensation (if more than 1 hour from my location )
10) Approximate date, time and duration of the shoot
11) Planned usage of the photos
12) Provisions for MUA, stylist, wardrobe, accessories, etc.
