Corollary Question:  How would you manage to put a camera up near the ceiling, pointing straight down, without photographing your legs?

I use a 3-step ladder, the kind with a little tray at the top for screws, etc. that I can lean against,  lean slightly forward and shoot from the model's side, at about her waist.  Worst case, if I can't keep my feet out of the picture, there's always cut-and-paste.
