At a high level...This  may be a heavy dose of both surface blur and gaussian blur together painted on the skin using a layer mask for creative control and some desaturation happening on a clipped or masked adjustment layer to take away the skin color and make it more black and white looking.

For the skin tones, try this technique from Calvin Hollywood to desaturate orange:

You can use surface blue to wipe out skin texture and make it look like plaster. D&B for highlights and shadows, or Topaz Detail (which is what I used here).
