When you send in Form VA (along with a CD or DVD) to the copyright office do you need to include additional forms & proof of age for models? It seems to me like that would be important.

no you do not need to send in proof of age for models.  When the image is created by you, you haev created a piece of intellectual property that you own, reguardless of the age of the model.  You won the copyright when you create the image. 

The only thing registering copyright does for you is gets the US government to recognize your ownership, which is required in the event of a copyright dispute. 

The age of the model doesn't have anything to do with registering the copyright.  It is only a factor when the person or company who wants to buy your copyright to publish the image and they want to make sure that the model is of legal age.  SO don't send in any modeling releases when you apply for copyright.

Tim hit most of it right.
Registering the copyright adds protection, but you do not NEED it to win a dispute.

Also, for nudes, Age does not matter anywhere.
Some companies may require the proof of age if you sell, but for a simple nude, no law really requires it.
