What else would you do to this photo? Im debating lightening the hands a bit.

I would blend the eyes better to the rest of her face. Right now it looks like you excluded the eyes from the retouching process...especially right eye (dark bags under the eye etc.
I also would darken the edges of the photo a bit kinda like a custom vignette.

I like it!  I agree the eyes need some more work.  Just enough to freshen them up a bit.  I wouldn't erase the creases completely though, just lightne them a bit. ^_^ And yes, that hands need some work.
